loop-uh c1f468ffc0 Refactor proxy admission control into dedicated arbiter 
Introduce EndpointAdmissionArbiter to centralize request scheduling and admission logic. Move queue management from ConnectionBroker to the new arbiter, simplify EndpointHealth admission to a static method called on grant, and replace admission release listeners with direct drain notifications. This consolidates the complex scheduling state machine into a single coordinated component.

Also add new tracking types (AdmissionTicketKey, ProxySchedulerLifecycle, EndpointVerdict) and refactor open slot management to work with the arbiter's scheduling pipeline.
