    mtproxy: handle RemoteClosed, PSK & adaptive fixes

    Add explicit RemoteClosed terminal reason and map it through status/diagnostics (include cooldown_ms). Split adaptive logic: add FailureNeedsTlsProfileRotation to restrict client-hello profile rotations and keep FailureNeedsRecipe for recipe escalation; guard ApplyAdaptiveRecipe to ignore non-recipe diagnostics. Synthetic PSK: add ClearSyntheticPskTickets and NoteSyntheticPskDataPathSuccess, arm cache only after data-path success and clear tickets on post-handshake failures; switch to EndpointKey(_endpointId) for offers. SessionPrivate now records mtproxy endpoint/use, reports connection failures and logs mtproxy admission delays. Update tests accordingly.
